Instructions for filling out the account opening forms are as follows:
 Customer must read, understand, and complete the following pages:

•   Risk Disclosure Statement:  Pages 2 and 3

•   Trader Information, Customer 1:  Page 4, Customer 2 (if any): Page 5

•   Trader Agreement:  Page 10

•   Managed Account Authorization:  Page 11      (Either confirms Customer is the only person signed to trade on the account, or gives  authority from the Customer to a person other than account owner to trade on the account)

•  Apex Trading Policies and Procedure: Page 12

•  Funding Your Account: Page 14

•  Signature Verification:  Page 15

After completion of the Account Opening forms either:
A)  Bring the completed paperwork into our office location
B)  Fax the completed form to one of our office locations:

Once you have completed the Account opening process, you will become an Apex FX Trading client, can fund your account, and begin trading using the MetaTrader Platform.
